Background: Although it is known that differences in paediatric primary care (PPC) are found throughout Europe, little information exists as to where, how and who delivers this care. The aim of this study was to collect information on the current existing situation of PPC in Europe. Methods: A survey, in the form of a questionnaire, was distributed to the primary or secondary care delegates of 31 European countries asking for information concerning their primary paediatric care system, demographic data, professionals involved in primary care and details of their training. All of them were active paediatricians with a broad knowledge on how PPC is organised in their countries. Results: Responses were received from 29 countries. Twelve countries (41%) have a family doctor/general practitioner (GP/FD) system, seven (24%) a paediatrician-based system and 10 (35%) a combined system. The total number of paediatricians in the 29 countries is 82 078 with 33 195 (40.4%) working in primary care. In only 15 countries (51.7%), paediatric age at the primary care level is defined as 0-18 years. Training in paediatrics is 5 years or more in 20 of the 29 countries. In nine countries, training is less than 5 years. The median training time of GPs/FDs in paediatrics is 4 months (IQR 3-6), with some countries having no formal paediatric training at all. The care of adolescents and involvement in school health programmes is undertaken by different health professionals (school doctors, GPs/FDs, nurses and paediatricians) depending on the country. Conclusions: Systems and organisations of PPC in Europe are heterogeneous. The same is true for paediatric training, school healthcare involvement and adolescent care. More research is needed to study specific healthcare indicators in order to evaluate the efficacy of different systems of PPC. [less ▲]

Different screening strategies are currently recommended to identify children with (familial) hypercholesterolaemia in order to initiate early lipid management. However, these strategies are characterised to date by low adherence by the medical community and limited compliance by parents and children. In a literature review, the authors assess which children should undergo screening and which children are in effect identifi ed through the currently recommended strategies. Furthermore, the authors discuss the different screening tools and strategies currently used in Europe and what is known about the negative aspects of screening. The authors conclude that currently recommended selective screening strategies, which are mainly based on family history, lack precision and that a large percentage of affected children who are at increased risk of future coronary artery disease are not being identifi ed. The authors propose universal screening of children between 1 and 9 years of age, a strategy likely to be most effective in terms of sensitivity and specificity for the identification of children with familial hypercholesterolaemia. However, this concept has yet to be proven in clinical practice. [less ▲]

We propose a new authenticated encryption scheme PAEQ, which employs a fixed public permutation. In contrast to the recent sponge-based proposals, our scheme is fully parallelizable. It also allows flexible key and nonce length, and is one of the few which achieves 128-bit security for both confidentiality and data authenticity with the same key length. The permutation within PAEQ is a new design called AESQ, which is based on AES and is 512 bits wide. In contrast to similar constructions used in the SHA-3 competition, our permutation fully benefits from the newest Intel AES instructions and runs at 2.5 cycles per byte if used as the counter-mode PRF. A large variety of content sharing applications rely, at least partially, on gossip-based dissemination protocols. However, these protocols are subject to various types of faults, among which selfish behaviours performed by nodes that benefit from the system without contributing their fair share to it. Accountability mechanisms (e.g., PeerReview, AVMs, FullReview), which require that nodes log their interactions with others and periodically inspect each others’ logs are effective solutions to deter faults. However, these solutions require that nodes disclose the content of their logs, which may leak sensitive information about them. Building on a monitoring infrastructure and on homomorphic cryptographic procedures, we propose in this paper PAG, the first accountable and partially privacy-preserving gossip protocol. We assess PAG theoretically using the ProVerif cryptographic protocol verifier and evaluate it experimentally using both a real deployment on a cluster of 48 machines and simulations. The performance evaluation of PAG, performed using a video live streaming application, shows that it is compatible with the visualisation of live video content on commodity Internet connections. Furthermore, PAG’s bandwidth consumption inherits the desirable scalability properties of gossip when the number of users in the system grows. [less ▲]

Vykdant BMC-EU projektą (Pagrindinės motorinės kompetencijos Europoje), buvo sukurta naudojimosi pagalbos ir pagalbos priemonių rinkinys, pagrįstas pagrindinių motorinių kompetencijų koncepcija ir duomenų rinkimo rezultatais pirmame projekto etape, ir kuris yra dalis trečio intelektinio produkto. Šiuo priemonių rinkiniu siekiama paskatinti jį įgyvendinti, ypatingai per reguliarias fizinio ugdymo pamokas, adaptuojant prie fizinio ugdymo mokymo turinio ir metodų. Pirmame etape buvo sukurta pagalbos sistema, skirta mokinių pagrindinių motorinių kompetencijų įvertinimui ir siekiant įvertinimo metu nustatyti stipriosioms ir silpnosioms pusėms. Remiantis šia sistema, buvo sukurti pagalbos priemonių rinkiniai, paruošti diegti konkrečiose ugdymo situacijose, kad būtų naudingi mokiniams, kuriems reikia pagalbos vystant pagrindinius motorinius įgūdžius. Pagalbos priemonių rinkinį sudaro MOBAK (pagrindinės motorinės kompetencijos) testų rezultatų aiškinimo gairės, MOBAK paramos sistema, žodynėlis, taip pat pagalbos priemonių rinkinys, apimantis priemones fizinio ugdymo mokytojams, MOBAK užduočių aprašymo kortelės ir MOBAK veiklų kortelės su fizinių veiklų variacijomis, remiantis į kompetenciją orientuotu požiūriu.
